Consumer Health Data Privacy Policy

 

Crescent Clearing, LLC, DBA Crescent Holistic Health Effective date: 8/21/2026 Last updated: 8/21/2026


This policy is provided under the Washington My Health My Data Act and comparable consumer health data laws, including Nevada SB 370. It is separate from our general Privacy Policy and covers only consumer health data. It must be linked separately from our homepage and must not contain information beyond what those laws require.
 

Categories of consumer health data we collect, and why

We collect the following categories of consumer health data:

  • Health conditions, symptoms, and status. Symptoms you report, including sleep, mood, energy, digestion, weight, temperature regulation, skin and hair changes, and pain; conditions you disclose; and your medical and family history.
  • Reproductive and sexual health information. Menstrual cycle information, perimenopausal and menopausal status, fertility information, pregnancy history, and libido.
  • Measurements of bodily functions and biological characteristics. Hormone panel results, including the DUTCH test, and any other laboratory results you order through us or share with us.
  • Use or purchase of medications and supplements. Prescriptions, over-the-counter medications, and supplements you take or that we recommend.
  • Health-related surgeries or procedures. Where you disclose them as part of your history.
  • Diagnoses or treatment you have received elsewhere, where you tell us about them.
  • Bodily functions and vital signs, where you report or track them.
  • Information about efforts to seek health services, including appointments you book and services you buy from us.
  • Information derived or extrapolated from the above, including the patterns and imbalances we identify and the protocols we build for you.


We collect this data for one purpose: to provide the health services you have asked us for, or to prepare to provide them. That includes assessing your situation, interpreting your laboratory results, building and adjusting your protocol, communicating with you about your care, and keeping the records the law requires us to keep.


We do not collect consumer health data for advertising, and we do not use it for advertising.


Categories of sources we collect it from

  • Directly from you — through online assessments and surveys on our websites, intake forms in our client portal, consultations and sessions, email, portal messages, and any document you upload or send us.
  • From laboratories, at your direction — hormone panel results returned by the testing laboratory after you order a test through us.
  • From a parent or legal guardian, where the client is a minor.
  • From another practitioner or provider, only where you have asked us to obtain records or asked them to send them.


We do not obtain consumer health data from data brokers, from purchased lists, or from advertising platforms.


Categories of consumer health data we share

We share consumer health data only where it is necessary to provide the service you asked for, or where you have directed us to.

The categories shared are:

  • Contact and identifying details together with the fact that you are a client, shared with the platforms we use to run the practice.
  • Health conditions, symptoms, history, laboratory results, and protocols, shared with our client portal and record-keeping platform.
  • Name, contact details, and the test being ordered, shared with the laboratory ordering platform and the testing laboratory.
  • Any consumer health data you specifically direct us to send, shared with the recipient you name.

We do not sell consumer health data. We have not sold it, and we will not. We do not share it with advertisers, advertising networks, social media platforms, analytics providers, or data brokers.

We may also disclose consumer health data where required by law, subpoena, or court order, or where necessary to prevent serious harm.


Your rights, and how to exercise them

You have the right to:

  • Confirm whether we are collecting, sharing, or selling your consumer health data, and to access that data, including a list of all third parties and affiliates with whom we have shared it.
  • Withdraw your consent to our collection and sharing of your consumer health data.
  • Have your consumer health data deleted, including from our archived and backup systems.

How to make a request. Email cynthia@crescentholistic.com with the subject line "Consumer Health Data Request" and tell us which right you want to exercise. You may also write to us at 11161 E State Rd 70, Unit 110-944, Lakewood Ranch FL, 34202.


Verification. Before we act on a request we will verify that you are who you say you are. For health data this normally means confirming details of your record that only you would know. We use the information you give us for verification for no other purpose.


Timing. We will respond within 45 days of receiving your request. If we need more time we will tell you why within that 45 days and take no more than a further 45 days.


Authorized agents. You may use an authorized agent to make a request on your behalf. We will ask for written proof that you authorized them.


Cost. There is no charge, and we will not treat you differently for making a request.

If we deny your request. We will tell you why in writing, and you may appeal by replying to that message. We will respond to an appeal within 45 days. If we deny the appeal, we will tell you how to complain to your state Attorney General.


What deletion means in practice. If you ask us to delete your consumer health data we will delete it from our active systems and instruct every third party listed above to do the same, and we will confirm when it is done. We may keep the minimum records the law requires us to retain, and we will tell you exactly what those are. If we delete your data we can no longer work with you, because the record is the work.
